| Registration Information |

Pre-registration
Required for all programs and should be done at least one week prior to activity
unless otherwise noted. Some programs are popular and fill very quickly.
A waiting list will be established after an activity becomes full.
Participants not residents within the Township are considered a nonresident.
Mail-in Registration
Print and complete the
registration form,
sign the waiver at the bottom and mail it to the Park District office with your
full payment.
Drop Slot Registration
Print and complete the
registration form,
sign the waiver at the bottom and drop it off with your full payment at the Park
District office drop slot.
Office Registration
Call or stop in the Park District office during regular business
hours. Visa and Mastercard Accepted.
Fax Registration
Complete your
registration form, sign the
waiver, complete the credit card payment information and fax it to the Park
District at (815) 562-5383.
Refund Information
The Park District will give full refunds if a program is cancelled.
Participant's cancellations that are two days or fewer prior to the program will
be assessed a $5 processing fee. No refunds will be given on or after the
day the activity is scheduled to begin. Refunds will be mailed to the
original payee; please allow two weeks for processing.
Cancellations
The Flagg-Rochelle Park District reserves the right to add, cancel or make
program changes as necessary.
